Privacy Policy
It is the policy of this Practice to provide quality healthcare services to all patients. Protecting the personal privacy and confidentiality of patients and their records is a very important part of our service to the community. Staff members are instructed to regard this policy as their primary job responsibility.
In 1966 the Federal Government enacted legislation known as HIPAA, the Health Insurance Portability and Accountability Act. In Privacy Rule provisions of this legislation, Federal Government expanded patient rights and data confidentiality. An individual has legal rights that apply to use and disclosure of his/her Personal Health Information (PHI).
As a patient treated by this facility you have the right to:
- receive a detailed copy of the Notice of Privacy Practices
- consent to and/or authorize use of your health information
- request restrictions on who can see or use your health information
- receive information from the practice in a confidential way
- review, copy and/or request to change your health information
- request a report of who has been given any of your health information
If you observe or experience any privacy violation, you have the right to file a complaint with the physician in charge of this office or with the Department of Health and Human Services (DHHS).
